It is like your gym membership or Netflix that you pay monthly fee and you get unlimited appointments and easy quick access to your primary care provider whenever you need.

Membership Plan

Age 4-18 years old:
$49/month

Age 19-64 yeas old:
$89/month

Age 65 years old and older:
$99/month

Family Plan: $220/month
for 2 adults and 1 child
Each additional child $30

Single Parent Plan: $130/month
for 1 adult and 1 child
Each additional child $30


Enrollment fee:
$50 per person
Maximum $150 for a family that enroll at the same time.
Second timer re-enrollment: $100
Third timer re-enrollment: $200
